QPA Accreditation means that our practice meets strict standards set by the Royal College of General Practitioners (RACGP), acknowledging our commitment to quality general practice patient care.

For your first appointment

Please arrive 10 minutes early and check in with our friendly front office staff and:

  • You may be required to fill in some health information forms
  • Download New Patient Forms below (1) New patient form (2) New patient medical form
  • Please bring your valid Medicare card if you have one
  • Please bring a valid Pension Card if you have one
  • Make a list of the current medications along with the doses you are currently taking

If you have two or more medical issues you would like to discuss please request a longer appointment by calling (03) 9372 0011.

Medicare rebates online

You can automatically claim your Medicare rebates via electronic claiming after your doctor’s appointment at our offices, and the rebate will go straight into your bank account.

To submit a Medicare claim online, you must be registered, have your Medicare card and your doctor’s account – Find out more here or ask one of our friendly front office team for more information.

Please pay your account at the conclusion of your appointment by:

  • Cheque
  • EFTPOS
  • MasterCard
  • Visa.

 Bulk Billing Eligibility Criteria

The following patients are eligible to have their appointments Bulk Billed:

  • All aged pensioners with a valid pension card
  • Children aged 15 and under with a valid Medicare Card.

Your Privacy

Your medical record is a confidential document. We maintain security of your personal health information. We ensure that this information is only available to authorised members of staff.

You have the right to access your health information and may request to view your medical records. A fee may apply and access to information in Victoria is covered under the Victorian Health Records Act 2001. If this is requested it will be scheduled at a time suitable to yourself and a doctor.

Complaints

We highly value your support and comments. Should you wish to make a complaint or are dissatisfied, with any aspect of your medical care or service provided by this medical centre, please discuss it with your doctor or the reception staff.

Patients are asked to submit their complaint in writing to the Practice Manager. Patients will be advised in writing of the progress of this complaint and kept up to date with all actions relating to the complaint.
